REQUIRED PROGRAM FORMS

This packet of forms must be filled out by all cadets at the beginning of each school year/start of an ROTC course.

Please download, complete all forms, and submit in Canvas once signed by all required individuals.

Please Note: The medical release form must be signed by a doctor if you answer "yes" to any of the questions.

If anything needs to be changed, please contact Captain Reed.

FIELD TRIP FORMS

Please complete these forms if you are going on a field trip with the unit. These forms will usually be given out in class but in the event you do not get one, they can be downloaded from here.
